Coverage can vary by plan, service, and visit. We encourage patients to confirm benefits with their insurance provider prior to appointments.
We provide complimentary insurance benefit verification as a courtesy. We will collect copays, deductibles, coinsurance, or other estimated patient responsibility based on the information provided by your insurance copmany at the time of serice. However, benefit information is not a guarantee of payment or coverage. Your insurance company ultimately determines coverage and patient responsibility, it is a contract between you and your insurance carrier.
We also offer cash rates for patients without insurance or for services not covered by your plan.
